With enormous problems getting the anesthetics required. Not to mention 4.5 watts of blue reduced to 3 watts out of the fiber when focused is on the low side for anything but certain kinds of dental surgery and internal coagulation. Its just not an optimal wavelength for soft tissue. Yes there are so called "skin surgery" devices sold illegally with overdriven blue diodes. What nobody tells you is they mainly generate scar tissue on the surface of the skin and more or less prevent future removal of the tats they are targeted for. There is a big difference between burning/scaring someone or doing actual useful surgery.
